Office Visit/New Patient

• Appointments are generally scheduled within 24hrs. However walk in appointments are available. Please contact office to check availability.
• New patients arrive 30 min and established patients 15 min prior to appointment time.
•  Bring all current medications , prior medical  records, current insurance card
• Copays are expected at time of service.
• Require 24 hour notice for appointment rescheduling or cancellation.
• New Patient forms: Download and bring on first visit.
